Complete Student Enrollment Forms:

To enter Kindergarten, a child must be 5 years old on or before September 1, 2024. To enter Transitional Kindergarten, a child must be 5 years between September 2, 2024 - June 2, 2025.

For Kindergarten or 1st Grade only, submit a completed Report of Medical Examination for School Entry (CHDP) if the student is new to the school system: download here...

Specific dates of immunizations must be included. Return this form to the school. The form is also available from most physicians, or any school office
